To set up an accommodation plan with Accessibility Services, please follow the steps below:
STEP 1
Complete the Online Intake Form.
The Intake Form serves as a student’s formal request for Academic or Housing services. They may complete this form at any point after acceptance to the College. The registration process takes time; therefore, all students are encouraged to register with Accessibility Services (AS) in a timely manner. AS recommend students submit request for services at least 4 weeks prior to the start of semester classes. Registering late in the semester can delay implementation of accommodations. Accommodations are not applied retroactively.
STEP 2
Submit disability documentation following the Accessibility Services Accommodation Policy.
Disability documentation is uploaded as part of the Online Intake Form. Alternatively, disability documentation can be submitted by mail or email. To ensure email confidentiality, use a LaGrange College student email address and send to Accessibility@gofuya.com.
STEP 3
The ADA Coordinator will review information and determine the student’s eligibility for services. Allow up to 10 business days for the Intake Form and documentation to be processed.
Once all information has been reviewed, the ADA Coordinator will send instructions on how to complete the accessibility services registration process to the LaGrange College student email.
STEP 4
Once eligibility of services has been determined, the student will receive instructions via a LaGrange College student email to schedule an Intake Appointment with the ADA Coordinator. This step is required to finalize student registration with Accessibility Services. The intake appointment consists of determining the accommodation plan and explaining AS policies and procedures.
